It would use the system of Missions from FFXI, for people seeking the depth and complexity of an actual storyline; there would be a vast and fulfilled PvP line of progression with arenas, objective-based battlefields, and competitions of various sorts including things such as footraces, scavenger hunts, and the like; there would be a full PvE game with quests attached that sometimes crossed paths with the Missions but were often tied to individual quest-givers and the character, while Missions are more factional/regional/global in scope;
lastly, there would be a crafting system in place that would be very deep, wherein players could make their own equipment (complex a la Vanguard/EQ2) or customize their existing equipment (simple, a few items and simple combinations, a la WoW) so that each player can interact with the system without having to slog through an epic amount of tedium -- homemade gear would be better of course, but more expensive to create and upkeep, so there's always a tradeoff.
It would probably be fantasy based but very low fantasy, medieval fantasy. Magic would be limited -- alchemy and mysticism, but it would be present. Martial combat's the name of the game, and political warfare is equally important.
