I loved the risk involved in playing the game. I can't remember how many times I got the shit ganked out of me and lost everything and I was able to start back up running around half naked in my deathrobe and get back on my feet. It was such a great feeling. Or starting a new guy to test a new character mix and discovering some great combination of skills. The social aspect was great, too. You'd meet people in dungeons fighting monsters and teamwork just kind of happened--until the PKs showed up and the real fight began, but it was still great.
So, in short, I would love to see a new non-WarCraft MMO game that actually has consequences. When you die, you lose most of your shit. People can screw people over by stealing or ganking, but there are reasonable penalties--criminals are hunted by law enforcement if they try to go into a town, and there are bounties and posses to worry about. The world is one big place, there are no private dungeon instances for you and your friends. Your character grows and develops over time.
