The game is an excellent throwback to the old days, yet adopts many new gameplay mechanics from today, without diluting the experience.
It's clunky in parts but not 'bad' at all. A few levels look a little bland, some polygons lacking, some texture detail and lighting passes - but that's only a few and nothing shockingly bad at all. In some sections I've been just outright wowed by the graphics actually.
The one liners, comedy and stuff has been easily 80% funny, there's 15% cringeworthy and 5% holy fuck did they just say that (also funny) (I liked the "I thought this was an out hole or one direction only?" or some kind of comment on the flesh asshole door things)
The 2 weapon thing isn't a showstopper.
Seriously, I really think anyone old enough to remember Duke who can look back fondly should pick this up and just switch off for a while. It's not perfect but it's significantly better than I expected and worth it for older gamers.
The young guys are either going to fucking detest it, or love it, I doubt there will be an inbetween.
(I must be about 3/4 of the way through)
