I was tempted to try it to play some new maps, but from what I can see in the map browser, it looks like no one is playing premium maps. I don't want to pay for it only to find out that every premium map server is empty. Anyone know if this is the case?

Let's ignore, for a second, the fact that UT was never used professionally, and had probably 5-10% the number of 3rd party mods that Q3 had.
Let's also ignore the fact that, at the height of its popularity, there were upwards of two MILLION people playing Quake 3 online, all at once (This, by the way, is more than any FPS before or since--because of market fragmentation it will likely never be surpassed).
You're still completely wrong about the shallow monotony. Quake was the opposite of shallow.
First, you had to learn what the weapons did, and work on your aim.
Second, you learned the maps.
Third, you learned how to strafe and rocket jump. (This was unique to quake, rocket jumping in UT4 basically meant death).
Fourth, you learned the timings of items and the sounds they made.
Fifth, you learned that players made different sounds depending on how damaged they were
Finally, you learned map control.
If you started playing TDM, you started learning lessons like: "The player who runs alone dies alone." And you had all of the competitive strategy of a modern FPS, but without voice chat.
The game was actually quite deep, despite it's humble, simple facade. The game is still fun every time I fire it up.
