The Millenium Girl jazzes things up by adding a Story mode, so you're not just dungeon-crawling, and slapping on new 3D graphics, animated cutscenes, and voices. Atlus describes The Millenium Girl as "an entry point to the series," which may be tempting if you're reluctant to jump into something which is now up to 4 entries.

I've heard that its the same basic formula, there is just more of those classic anime cutscenes with janky dialogue with two heads talking back and forth. IV was pretty light on that stuff. Besides that the combat is supposed to be almost exact.
