Donator T-Shirts Form

Ok folks, last call for donator T-Shirts. If you were one of the folks who sent in donations of $25 or greater and wish for $15 of that donation to now go towards scoring yourself a T-Shirt. Fill out this form. Only fill out the form if you want a shirt. If you have filled out the form last time this was mentioned then there is no need to fill it out again. In addition, since many people have requested it. If you wish to actually pay separately for your T-Shirt You can just go here and submit $15 or whatever you wish. Only people who donated can get these shirts. Do not submit this form if you did not previously donate. If you did not donate and want to get a shirt, go here. . Thanks everyone for helping out. Also, a new design for the shirts is in the works. If you have any serious idea submissions feel free to email them to me.
